Job Description:
A company is looking for a Remote Care Coordinator (Certified
Medical Assistant).
Key Responsibilities
Conduct monthly care coordination calls with patients and
healthcare providers
Create and revise personalized care plans in collaboration with
healthcare practices
Identify social determinants of health and coordinate assistance
for patients
Required Qualifications
At least 1 year of experience as a Certified or Registered Medical
Assistant
Active medical assistant accreditation required
Experience with multiple EMR/EHR systems
Ability to work a 40-hour schedule, Monday through Friday
Private home office setup with fast and reliable internet is
required
