Clinical Resource NurseLPN(Hybrid)Our mission to change skin and

develop a skin condition requiring expert medical care. We believe

wound care advanced practice providers bring education and
encouragement to the people who take care of our patients 24/7 and
our Clinical Resource Nurses support our providers and partners.
When they get better at their jobs, our patients wounds heal faster
and that is our goal!* This is a behind the scenes position that
supports the relationship and function between the facilities staff
and UWH.*Actual hands-on wound care is minimal. The focus is
education and administration.Main Responsibilities (may include but
are not limited to):Conduct in-person and virtual facility
check-ins with the DNS, wound team, and/or SNF representative
regarding how we can best support the facility wound teamAddress
concerns raised by UWH providers and facility membersSet up new
facilities and present new partners with UWH protocol and
educationShare knowledge through education and training to ensure
quality and efficiency of Team-Centered Skin & Wound Care in
current partner facilities; this may be completed in-person through
small group powerpoint educations or via Teams meeting (when
necessary)Support facilities with skin audits and skin reviews on
UWH patientsRound with new partner facilities and accompany new
providers for first roundsSupport and build relationships with
providers through semi-annual rounds with the wound teamsTravel to
local facilities on a regular basis and extended travel on an
as-needed basis, which may include overnight stayAttend skills
fairs, conferences, and trade fairs as a representative of
UWHAttend regularly scheduled team meetings, Grand Rounds, for
continued training, team building, and leadership
developmentAccomplish other tasks as assignedSkills required to
succeed:LPN licensed in working stateThis fast-paced position
requires standing for extended amounts of timeDriving! You must
have reliable transportation; valid drivers license and current
auto insuranceHybrid position - some remote work available from a
home office1-2 years of wound care experience required1-2 years of
Skilled nursing/Long term care experience requiredAbility to work
well within a team of healthcare providers and partner
facilitiesProficient in MS Office (specifically Teams, Outlook,
Excel, Powerpoint, and Word)Must be self-motivated, punctual,
dependable, and able to work/schedule appointments
independentlyAbility to respond appropriately to constant change in
priorities and servicesAcute attention to detail with a strong,
self-sufficient work ethicExcellent organization and use of time
management skillsMust have strong interpersonal skills and the
ability to communicate effectively with a variety of people through
presentations and verbal/written communication linesMust be
trustworthy, honest and have a positive and professional
attitudeCompensation: 75,000.00 92,000.00 DOE with bonus
potentialTerritory: North Seattle, Everett, Bellingham as
neededThis position is classified as: Salary, Exempt; Full-Time
employmentHours: Typical hours are Monday through Friday; schedule
varies between the hours of 6:00am 5:00pm, no nights, weekends, or
